At Health Sprout Nutrition, we strive to make our services accessible to all. We are pleased to accept Medicare Chronic Disease Management (CDM) referrals, Private Health Insurance and DVA rebates, which can significantly reduce out-of-pocket expenses for eligible patients.
Private Health Insurance
Many private health insurance funds offer rebates for dietitian services. The level of coverage varies depending on your policy, so we recommend checking with your provider for specific details. After your consultation, you will receive a receipt with the necessary information to make a claim directly.
Chronic Disease Management Plan
The CDM plan is designed to help patients with chronic conditions receive comprehensive care. Under this program, eligible patients can receive up to 5 allied health services per calendar year. To qualify, you must:
Have a chronic condition that has been present or is likely to be present for 6 months or longer
Have a GP Management Plan and Team Care Arrangement in place
Be referred by your GP
Department of Veterans Affairs
Health Sprout is an approved provider for DVA. Gold Cardholders can access consultations at no cost. We also offer home visits if needed. If you qualify, please obtain a referral from your doctor before scheduling an appointment.
Cancellation Policy
At Health Sprout Nutrition, we recognise that life can be unpredictable, and appointments may need to be rescheduled or canceled. To accommodate all clients while maintaining high service standards, we have established the following cancellation policy:
Appointment Cancellations and Rescheduling: We require at least 24 hours' notice for any cancellations or rescheduling to allow us to offer the time slot to other clients. Please contact us via phone or email for any changes.
Late Cancellations and No-Shows: Cancellations made with less than 24 hours' notice will incur a cancellation fee, while no-shows will be charged the full consultation fee. This policy helps us maintain availability for all clients.
Emergency Cancellations: We understand emergencies can arise. If you need to cancel due to an emergency, please contact us as soon as possible; we will consider these situations on a case-by-case basis and may waive the cancellation fee at our discretion.
